Call your child’s school and ask for the nurse. Nurses serve more than one building, but the school secretary can connect you to the school the nurse is at.

Students should be kept home when they cannot participate in the classroom. Symptoms which would prevent this would be fever, diarrhea, vomiting, eye drainage, communicable illness, etc. Students should be fever free for 24 hours without the use of fever reducing medication before returning to school.

If your child has a health diagnosis, such as diabetes, asthma, allergies, seizures, or other health diagnoses, an Individualized Health Plan must be completed annually by you and your child’s physician. Please contact the Health Services team to request an Individualized Health Plan. We recommend your physician fax or mail the form directly to the school. The school nurse is available to assist you in completing the form or answering any question that you may have concerning the use of the information requested.

It is important that you notify the school nurse during the school year of any changes in your child’s health status, medications, treatments, or physicians. Parent/school communication is vital to the successful care and the safety of our students. This information assists us in providing a successful school experience for all students with special challenges or health concerns. 

This Individualized Health Plan will be used to plan for the care and safety of the student. It will be shared with only those members of the professional school staff who have direct responsibility for the student when in school or participating in school activities. When necessary the school nurse will provide in-service programs for teachers, staff, and bus drivers regarding the health condition and the specific emergency procedures that may need to be initiated.

If it is necessary to give medication at school either daily, or as needed, a parent signature is required. Forms are available at school. All medication must be in the small original bottle, and transported by an adult (students in grades 9-12 may carry if needed).

Yes. The nurse, principal, or school secretary will contact you. That is why it is so important for school to have your up-to-date contact information. Please notify the office of any changes. First aid will be applied while someone is contacting you. If an ambulance is required, you will be contacted immediately.

School guidelines are followed according to the Indiana State Department of Health's Immunization Guide. This guide can also be requested at school.

The Indiana State Department of Health (ISDH) requires vision screening in grades K, 1, 3, 5, & 8. Nurses will also do any vision referrals. Hearing screens are done by Wabash-Miami Area Program staff in grades K, 1, 4, 7, & 10.

Head checks will also be done on an individual basis as needed as fall and spring seasons find head lice a common nuisance.
